An elevated ADA level in pleural fluid is a possible sign of tuberculosis. ADA levels in pleural fluid are high in a person with signs and symptoms of tuberculosis, although this may be due to another cause. Some people have pleural fluid due to other reasons, such as a bacterial infection or the effects of an antimicrobial.

The ADA test is an alteative to culture, which is the gold standard for detecting M. tuberculosis in pleural fluid. However, culture tests are time-consuming and require a high enough number of bacteria to accurately diagnose a patient with TB. However, pleural fluid is unique in that it may contain a large volume of fluid and little or no bacteria.  Therefore, ADA results are useful in guiding treatment until culture results can be determined.

The LD/ADA ratio of a TB pleural effusion has been described in multiparameter scoring systems. It was lower than that of non-TB pleural effusion, which suggests that ADA has a diagnostic role. This new biomarker can be used for monitoring inflammatory conditions in growing pigs. Pigs are typically housed in group conditions and may suffer from infections. It is also easier to detect a sick individual in the early stages of a disease because saliva sampling is less stressful.
